Special Information for Students

Many students have dental plans from their parents. For us to accept
these plans, we need the following information.

1. Parent's employer
2. Insurance company/carrier.
3. Plan number with parent's ID number.
4. Parent's birthdate and work phone number.
5. Proof that you are a student.

Many students forget number 4, so please remember.

Please be aware that NAIT students on the NAITSA dental plan should only attend one office. There is also a missed appointment charge if appointments are missed without 24 hours notice.
